Pet Therapy Handler Scholarship Application

*Note:  You must be at least 12 years old to participate in the Pet Handling training.  Junior applicants (ages 12-17) are required to provide their parent's phone and email information.

We respect your privacy. Your information will only be shared with members of the St. Francis of Assisi Pet Ministry scholarship committee. Once we receive your completed form, we will reach out to you for more information and to answer any questions you may have.

The Quad City Dog Obedience Club and CM Academy of Dog Training reserve the right to discontinue training if deemed necessary.
